A formulary request ($65 flat rate fee) entails answering a questionnaire to be used to create a singular tailor-made herbal formula to address a specific concern (tincture, bath, tea blend, smoke blend, etc). Crafting or procurement of herbal goods is not included, that is a separate cost based on the needs of the consulted and the costs of materials.
